Associates must be 18-64 years old, work 30 hours/week, hired on or after 9/20/2023 and apply during the open enrollment period: October 3, 2024 to October 25, 2024 to be eligible to apply with modified guaranteed issue underwriting meaning limited health questions. All other eligible associates hired prior to 9/20/2023, may enroll with full underwriting before coverage is issued.
